Job Description

Seeking a Weekend Home Health RN with a salary of $65,000-$75,000/year.

Responsibilities:

  • Provide home-based care post-hospitalization or rehab
  • Collaborate with physicians and multidisciplinary teams
  • Assess patients, document progress, and communicate changes
  • Educate patients and families on care plans, medications, safety
  • Coordinate discharge and ensure care goals are met

    • Requirements:

  • Graduate of an accredited RN program
  • Active RN license in South Carolina or multi-state license
  • At least one year of nursing experience, ideally in home health, Med/Surg, or Critical Care
