Job Description

Seeking a Nurse Practitioner for PRN home risk assessments, with new grads welcome and a $1,500 sign-on bonus.

Join a team providing in-home health evaluations to improve patient outcomes and close care gaps.

Responsibilities include conducting adult/geriatric assessments, collaborating with healthcare professionals, and providing follow-up care in homes, skilled facilities, or community settings.

Earn $1,900–$9,500/month based on assessments completed, with flexible hours including days, evenings, nights, and weekends.

Qualifications: Master’s degree or equivalent, current RN and NP licensure in Ohio, board certification, CPR/BLS/ACLS, and computer proficiency. New grads encouraged to apply.

Travel required to patient locations; valid driver’s license and insurance needed.
