Conduct comprehensive assessments up to one hour, reviewing health, history, medications, and social factors to improve care and close health gaps.
Work collaboratively with healthcare teams in home, SNF, and community settings.
Flexible hours including days, evenings, nights, and weekends.
Earn $2,300 to $11,500 per month based on assessments completed.
Qualifications include current RN and NP/PA licensure, board certification, CPR/BLS, and ability to travel.
Benefits include mileage reimbursement and limited voluntary benefits; no benefits for PRN except 401(k).
