Job Description

Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments in New York. New grads are welcome, with a $1,500 sign-on bonus.

Conduct comprehensive assessments up to one hour, reviewing health, history, medications, and social factors to identify care gaps and improve outcomes.

Work collaboratively with healthcare teams, providing follow-up and patient education in homes, SNFs, and community settings.

Earn from $2,300 to $11,500/month based on assessments completed, with flexible scheduling including days, evenings, nights, and weekends.

Qualifications include a Master’s degree or equivalent experience, current licensure, certification, and ability to travel.
