Job Description

We are hiring a Licensed Practical Nurse (LPN) to provide quality home health patient care, perform skilled nursing tasks, and support patients and their families.

  • Deliver services within the scope of practice, following the plan of care, and collaborating with the healthcare team.
  • Educate families on patient care and maintaining a healthy environment.
  • Monitor, document, and report patient responses and condition changes daily.

    Requirements include current LPN licensure, CPR certification, a valid driver’s license, vehicle insurance, and reliable transportation.
