Job Description
This RN Home Health position is ongoing, offering 40 hours weekly with flexible shifts and a staff employment type.
Major responsibilities include assessing patient needs, developing and evaluating individualized care plans during twice-daily visits, and involving patients and families in care. The nurse must recognize and respond to changes in patient condition, coordinate discharge plans, and implement evidence-based practices.
Key duties involve promoting safety, collaborating with healthcare teams, delegating appropriately, and participating in quality improvement. The role requires strong communication, critical decision-making, and time management skills, along with the ability to work independently in patients’ homes.
Qualifications include an active RN license, BLS certification, a valid driverâs license, and at least two years of recent acute care or emergency nursing experience.
