REF:#12f529ec2bb408b9 Welcome to Sisters of Divine Providence/St. Joseph Center, a privately funded and operated care center for the aging religious. We are currently seeking full-time and part-time LPN’s (ALL SHIFTS) to join our team and help us provide high-quality and compassionate care to our patients. You will be in helping to ensure that all patient care is provided in a safe and efficient manner while maintaining patient-focused care and compliance with regulatory requirements. Responsibilities

Maintains updated assessments on all residents in need of nursing care.
Maintains complete and confidential written and oral communications.
Keeps Mar and Care Plan updated as needed.
Makes appointments with physicians and scheduled appropriate testing.
Maintain accurate and complete medical records for all patients
Keeps medication orders up-to-date and provides safe administration of medications.
Maintains professional relationships with pharmacies for prompt and accurate delivery of medications and other ordered supplies.
Assists with the orientation of new staff.
Works as a member of the health care team and as a licensed staff assists other members of the team in completing assignments as needed.
Communicates pertinent information to the Nursing Director, Physicians and staff.
Maintains professional and positive attitude and fosters a “team spirit.”
Available for call-in if needed.

Requirements:

Current LPN license in the state of Pennsylvania
Strong physical, mental, and social assessment skills
Ability to work effectively in an ever-changing environment
Knowledge of regulatory requirements and best practices in healthcare
Minimum of 2 years of work experience in acute care and/or long term care nursing
