POSITION SUMMARY:<\/p>\n
In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services. Located in McAllen, Texas.<\/p>\n
KEY RESPONSIBILITIES:<\/p>\n
Responsible for establishing the Tapestry Health patient relationship for our chronic care management services.<\/p>\n
Be available at a minimum of 12 working days per month to conduct approximately 375 clinical visits onsite.<\/p>\n
Document patient encounters accurately and thoroughly within our platform at time of visit.<\/p>\n
Provide end of day report to program administrators to track progress for completion of visits.<\/p>\n
MINIMUM REQUIREMENTS:<\/p>\n
Qualified MD\/DO<\/p>\n
Active, unrestricted, Medical License in Texas<\/p>\n
Excellent clinical judgment and decision-making skills.<\/p>\n
Must have reliable transportation to travel to assigned buildings.<\/p>\n
Commitment to providing patient centered care and improving health outcomes.<\/p>\n
Familiarity with electronic health record (EHR) systems and healthcare technology.<\/p>\n
Ability to travel within 1-2 hours of assigned region<\/p>\n
COMPENSATION STRUCTURE:<\/p>\n
$25 per patient visit, paid upon completion of assigned facility census.<\/p>\n
