Job Description
Seeking a Weekend Home Health RN offering $65,000-$75,000/year with a supportive, nurse-focused culture.
Provide home-based care to patients post-hospital or rehab, managing illnesses and chronic conditions.
Work with physicians and multidisciplinary teams, assess patients, document progress, and report changes.
Educate patients and families on care plans, medications, safety, and recovery goals.
Ensure successful discharge and coordinate ongoing care.
Must be a graduate of an accredited RN program with an active license in South Carolina or multi-state license.
Minimum one year of nursing experience, preferably in home health, Med/Surg, or Critical Care.
