Job Description
Seeking a travel RN in Case Management for a 13-week assignment in Danville, VA, working 40 hours/week on days.
Develop and implement safe, individualized discharge plans based on clinical needs and coverage.
Collaborate with healthcare teams to ensure smooth patient transitions and prevent readmissions through effective coordination.
Conduct utilization reviews to verify medical necessity and resource use, ensuring compliance with CMS, Medicare/Medicaid, and Virginia Medicaid guidelines.
Communicate with insurers to obtain authorizations and maximize reimbursement.
Educate patients and families about care plans, discharge processes, and community resources.
Document assessments and plans in clinical systems and participate in interdisciplinary meetings.
Minimum 2 years of acute care discharge planning experience required.
