Job Description
Seeking a travel RN Case Management for a 13-week assignment in Fort Defiance, AZ, working 8-hour days, 40 hours/week.
Serve as primary RN case manager in a busy outpatient primary care setting.
Collaborate with providers and nursing staff to develop care plans, monitor outcomes, and coordinate follow-up care.
Advocate for patients, especially with chronic and preventive needs, and act as a liaison among patients, providers, and community resources.
Float to other departments based on qualifications and experience, assisting with patient education and interdisciplinary communication.
Requires active RN license (AZ or compact) and minimum 3 years recent case management experience.
