Job Description
Seeking a travel RN Case Management for a 13-week assignment in Torrance, CA, working 40 hours/week on days.
Perform bio-psychosocial assessments, document patient needs, risks, and vulnerabilities.
Collaborate with care teams for planning and coordination.
Manage discharge planning, home health, and care transitions.
Assist with admissions, needs assessments, and referrals to community resources like Medicaid, social services, and housing.
Ensure documentation aligns with evidence-based guidelines, including medical necessity and care goals.
Maintain records of family meetings, care conferences, and resource referrals.
Conduct PASRR screenings, handle 51/50 holds, and process appeals/denials.
Provide education, emotional support, and advocate for patients facing homelessness, mental health crises, or substance use challenges.
Manage a caseload of up to 25 patients, with a minimum of 2 years acute care experience, prior travel experience, proficiency in EPIC, and MSW certification.
