Team Manager, Hospice Home Care

Job Description

Manages and coordinates interdisciplinary hospice teams to deliver comprehensive care aligned with patient needs and organizational policies. Ensures implementation of quality improvement initiatives, regulatory compliance, and effective documentation. Participates in team meetings, committee work, and quality activities to promote best practices and professional standards. Oversees daily operations, including caseload management, productivity, and fiscal targets, while supporting staff training and performance evaluation. Facilitates collaborative care planning and clinical assessments covering medical, social, emotional, and spiritual needs. Performs managerial duties such as hiring, budgeting, and staff development. Requires a Registered Nurse license, a Bachelor’s Degree in Nursing, and at least three years of hospice experience. Strong communication skills and clinical management experience are preferred. Salary ranges from USD $98,200 to $130,800 annually.
