Job Description
Seeking a Registered Nurse (RN) Home Health Case Manager in Milwaukee, WI for ongoing, full-time (40 hrs/week) day shifts.
Responsibilities include managing caseloads, conducting initial assessments, creating and updating care plans, providing direct patient care, and coordinating with physicians and families.
Qualifications: Associate’s Degree in Nursing, 3+ years of clinical experience, RN license, BLS certification within 6 months, valid driverâs license, and a registered vehicle.
Skills needed: strong communication, interpersonal, organizational, and clinical judgment skills, with the ability to assess patient needs across all ages.
Physical demands include lifting up to 50 lbs, mobility in various environments, and exposure to weather and hazards.
