Job Description
Seeking a Registered Nurse (RN) Home Health Case Management for Sheboygan, Wisconsin.
Full-time, 40 hours/week, dayshift, ongoing staffing position.
Manage patient caseloads, conduct assessments, develop and adjust care plans, and provide direct patient care.
Communicate with physicians, coordinate discharge planning, and ensure documentation accuracy.
Must have RN license, BLS certification within 6 months, and a valid driver’s license.
Require associate’s degree in nursing and 3+ years of clinical experience.
Strong communication, organizational, and clinical skills, with ability to work with diverse populations.
Physical requirements include mobility, lifting up to 50 lbs, and working in varied environments.
