Seeking a Registered Nurse (RN) for ongoing Home Health Case Management in Westmont, Illinois.

40 hours/week, day shift, staff position.

Develops therapeutic nurse/patient relationships, provides individualized, holistic care, and assesses patient/family needs.

Ensures quality, cost-effective care by mobilizing resources, adhering to evidence-based standards, and participating in quality improvement.

Educates patients, families, and staff, adapting teaching to individual readiness.

Uses nursing process for clinical decision-making, plans discharges, and coordinates interdisciplinary care.

Demonstrates leadership, effective communication, and ongoing professional development.

Requires RN licensure, at least 1 year medical-surgical experience, current CPR, and good communication, documentation, and organizational skills.

Must be able to work flexible hours, respond quickly, lift up to 35 pounds, and travel to patient residences.
