Job Description
Seeking a Registered Nurse (RN) Home Health Case Manager in Park Ridge, Illinois, for an ongoing full-time day shift role.
Provides comprehensive home care to the homebound population, supporting patient independence and preventing rehospitalization.
Assesses, plans, implements, and evaluates nursing care, building strong patient relationships.
Collaborates with multidisciplinary teams and documents care via point of care systems.
Requires a nursing degree from an accredited school, one year of medical-surgical experience, Illinois RN license, and CPR certification.
Must be able to lift up to 35 pounds and assist with patient transfers and mobility.
