Job Description

Seeking a compassionate Registered Nurse (RN) Hospice for an ongoing staff position in Bay City, Michigan.

As a Hospice RN/Case Manager, you will report to leadership, manage patient care, serve as liaison with families and physicians, and collaborate with an interdisciplinary team.

Responsibilities include providing direct care, educating caregivers, evaluating patient needs, maintaining records, and participating in quality improvement.

Requirements include an RN license in Michigan, 1+ years of hospice or related experience, a valid driver’s license, and CPR certification.
