Job Description
Seeking a compassionate Registered Nurse (RN) Hospice for an ongoing staff position in Frackville, PA.
Provide direct patient care and support to hospice patients and families
Act as primary liaison between physician, patient, family, and care team
Manage a caseload, educate caregivers, and develop care plans
Collaborate with interdisciplinary team members including LPNs, CNAs, and support staff
Maintain clinical records and participate in quality improvement programs
Requirements include RN license in PA, 1+ years hospice or related experience, CPR certification, and a valid driver’s license
Candidates should demonstrate compassion, flexibility, and ability to support patients at home
