Seeking a compassionate Registered Nurse (RN) Hospice for an ongoing staff position in New Castle, PA.

As a Hospice RN Case Manager, you’ll report to the appropriate leader, manage patient care, and serve as the key liaison between physicians, patients, families, and the care team.

Responsibilities include providing direct patient care, educating caregivers, collaborating with interdisciplinary staff, evaluating needs, and maintaining clinical records.

Requirements: RN license in PA, 1+ years of hospice or related experience, CPR certification, a valid driver’s license, and a compassionate, flexible approach.
