Job Description

Seeking a compassionate Registered Nurse (RN) Hospice for an ongoing staff position in Dayton, Ohio.

As a Hospice RN Case Manager, you will report to the administrator or care manager, manage patient care, serve as liaison between physicians, patients, and families, and provide direct nursing services with a focus on comfort and dignity.

Responsibilities include evaluating patient needs, developing care plans, educating caregivers, collaborating with an interdisciplinary team, maintaining records, and participating in quality improvement programs.

Requirements: RN license in Ohio, 1+ year hospice or related experience, compassionate nature, driver’s license, CPR certification, and flexibility to support patients wherever they call “home.”
