Job Description
This ongoing RN Hospice position in Roswell, Georgia, involves providing on-call, direct patient care to hospice patients and supporting their families beyond regular hours.
Reports to the Administrator or Care Manager and serves as the primary liaison between physicians, patients, families, and the hospice team.
Manages an interdisciplinary team including LPNs, CNAs, and support staff to ensure quality care.
Evaluates patient and family needs, develops and implements care plans, and maintains clinical records.
Participates in quality improvement programs.
Requires a current RN license, with 1+ years of hospice, home health, or oncology experience preferred.
