Job Description
Seeking an RN Hospice Case Manager in Augusta, Georgia for an ongoing staff position.
Provide direct patient care and support to hospice patients and families, acting as primary liaison with physicians and the care team.
Manage a caseload, educate caregivers, and collaborate with interdisciplinary staff to ensure comfort, dignity, and peace.
Maintain clinical records and participate in quality improvement programs.
Requirements include compassion, flexibility, RN license in Georgia, 1+ years of hospice or related experience, current CPR, driver’s license, and insurance.
Preferred experience includes ICU, ER, geriatric, or critical care backgrounds.
