Job Description
Seeking a Registered Nurse (RN) for ongoing Home Health Case Management in Elmhurst, Illinois.
40 hours/week, dayshift, staff position.
Develops and maintains therapeutic nurse/patient relationships, assesses patient and family needs, and provides individualized holistic care.
Ensures quality, cost-effective care by mobilizing resources, participating in quality initiatives, and adhering to standards.
Educates patients, families, and staff, assessing learning needs and promoting growth.
Uses nursing process for clinical decision-making, prioritizes patient care, initiates referrals, and coordinates interdisciplinary care.
Demonstrates leadership, collaborates with team, participates in professional development, and maintains current knowledge.
Requires Illinois RN license, CPR certification, one year med-surg experience, and excellent communication skills.
Must be physically capable of lifting up to 100 pounds, working in varied conditions, and traveling within service areas.
