Job Description
Seeking a Registered Nurse (RN) for a Home Health position in Mentor, Ohio.
This ongoing, full-time role involves providing high-quality, personalized care in patients’ homes.
Responsibilities include assessing patient needs, developing and updating care plans, communicating with the healthcare team, and ensuring privacy compliance.
Requirements: current RN license, 1+ year clinical experience, ability to travel within the area, dependable vehicle, and valid driverâs license.
Preferred qualifications include prior home health or hospice experience, and familiarity with HomeCare HomeBase.
