Job Description
Seeking a Registered Nurse (RN) Home Health for ongoing full-time work in Greenwich and Bridgeport, Connecticut.
Provide patient-centered care in the home setting, assessing needs, developing care plans, and ensuring quality treatment.
Maintain confidentiality and adhere to HIPAA regulations.
Evaluate patients’ physical, psychological, social, and spiritual health, adjusting care plans as needed.
Communicate findings and coordinate with healthcare team members to ensure safe and effective care.
Require current RN license, 1+ year clinical experience, reliable vehicle, and willingness to travel 50%.
Preferred: Bachelor’s degree, prior home health or hospice experience, and familiarity with home care software.
