Seeking a Registered Nurse (RN) Case Manager, Hospice for an ongoing staff position in Elkhart, Indiana.
Provide high-quality hospice care in patients’ homes, assessing physical, psychosocial, and spiritual needs.
Communicate findings, update care plans, and collaborate with interdisciplinary teams.
Educate patients, families, and staff on hospice philosophy and symptom management.
Perform bereavement support and coordinate care delivery.
Require an accredited nursing degree, current RN license, 1+ year clinical experience, and 50% travel.
Must have valid driver’s license and reliable transportation.
