Job Description
Seeking a Home Health Speech Language Pathologist Care Manager to coordinate patient care in a home health setting.
Perform evaluations, develop and adjust personalized care plans, and ensure effective speech therapy interventions.
Educate patients, families, and caregivers on speech improvement strategies and adaptive equipment.
Collaborate with interdisciplinary teams, document treatment and progress accurately, and monitor patient outcomes.
Qualifications include a Master’s in Speech Therapy, relevant clinical experience, state certification, and a valid driver’s license.
Preferred experience includes Medicare-certified home health, OASIS certification, and case management credentials.
