Job Description
Monitors and assesses home health field services, supervising Home Health Aides and providing case management, clinical intervention, care plan development, and follow-up.
Conducts initial and ongoing home visits for patient assessment, training, and crisis management, reporting significant changes to the clinical team.
Collaborates with physicians and care teams to gather medical history and medication details.
Ensures HHA adherence to guidelines during assessments and identifies training needs, documenting electronically.
Provides start-of-care overview to patients and caregivers, educates HHAs on care plans, and participates in quality assurance and special projects.
Qualifications include a NY RN license, CPR/BLS certification, with at least two years of clinical nursing experience, preferably in home care settings. Salary ranges from $77,200 to $96,500 annually.
