Provides intermittent home nursing services under physician-approved care plans, reporting to a Clinical Manager and supervising Home Care Aides.

Requires RN licensure in Connecticut, a nursing degree, and at least 1 year of medical/surgical experience, with community health preferred.

Performs assessments, develops care plans, and evaluates patient progress, ensuring safety and quality standards.

Documents services accurately, coordinates with physicians and team members, and manages caseload efficiently.

Supervises and orients Home Health Aides, maintaining compliance with regulations.

Maintains professional skills through ongoing education, adheres to policies, and supports agency programs.

Works in patient homes and office environments, handling physical demands and working under stressful conditions, with reliable transportation.
