Job Description

This RN position in the Thoracic Surgery outpatient department involves providing collaborative, patient-centered care, including triage, education, care planning, and coordination across settings.

  • Supports self-management by assessing patient needs and promoting health literacy.

  • Advocates for patients by coordinating services, reviewing medications, and accessing community resources.

  • Educates patients and families, ensuring understanding through teach-back, and performs telephone triage.

  • Develops and updates individualized care plans, facilitating communication among healthcare providers and coordinating transitions, admissions, and referrals.

  • Provides coaching, counseling, and diagnostic testing, while ensuring regulatory compliance and maintaining accurate documentation.

  • Collaborates with interdisciplinary teams, supports quality improvement, and manages equipment and safety standards.

  • Qualifications include RN licensure, CPR certification, and preferably ambulatory care experience; skills in communication, patient advocacy, and clinical procedures are required.
