Job Description
This RN position in the Thoracic Surgery department supports outpatient care, providing patient assessment, education, advocacy, and care coordination.
Supports self-management by enhancing health literacy and conducting needs assessments.
Acts as patient advocate, coordinating services and verifying medication/orders.
Provides age-appropriate education, performs triage, and develops individualized care plans.
Coordinates transitions, referrals, home care, and follow-up, ensuring continuity of care.
Counsels patients and families, answers questions, and performs diagnostic tests.
Utilizes nursing process for assessment and updates care plans routinely.
Collaborates with interdisciplinary teams, supports compliance, and maintains documentation.
Qualifications include RN licensure, CPR, and preferred ambulatory experience. Skills include communication, computer proficiency, confidentiality, and medication administration. The role requires standing, walking, and adaptability in a clinical setting.
