Registered Nurse (RN) Case Manager PRN/Hourly position available, working M-Sun 8a-430p.
The role involves coordinating patient care, ensuring payer requirements are met, and developing individualized care plans in collaboration with an interdisciplinary team.
The ideal candidate has a current RN license, associates or bachelorâs degree, BLS certification, and at least 3 years of acute care experience or significant case management background.
Preferred qualifications include prior case management experience and certification.
