REF:#2bb0438f2818cad7 The Home Health Registered Nurse is responsible for traveling to a patient’s home to provide skilled nursing service in accordance with the orders of the patient’s health care professional, under a plan of treatment established by the healthcare team.Principal Responsibilities
Be responsible for the observation, assessment, nursing diagnosis, council, care and health teaching for patients, and health maintenance and illness prevention.
Provide progress notes to the patient’s provider when the patient’s conditions change or there are deviations from the plan of care, or at least every 60 days.
Direct the activities of the licensed practical nurse and supervise the aide in the home.
Administer medications and treatment as prescribed by the patient’s provider.
Act as the coordinator of the healthcare team to maintain the proper linkages within a continuum of care.
Perform acts in accordance with the IL Nurse Practice Act.
Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
Attends all training and educational requirements for role.
Education/Experience
1+ year experience in a clinical care setting as a nurse
Associates Degree or Bachelor’s Degree in Nursing is preferred
Previous Home Health or Hospice Experience is preferred
Licenses/Certificates
Current, unrestricted state RN licensee
CPR certification with American Heart Association
Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
Knowledge, Skills and Abilities
Able and willing to travel within branch/office coverage area.
Working Environment
This job operates in a professional office environment. Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
