The Float Registered Nurse is responsible for collaborating with a group of primary nurses to provide help with case management in the community and facilities. This includes completing skilled nursing assessments with comprehensive, routine and acuity visits. You will work collaboratively with the primary team and interdisciplinary teams to achieve high quality patient standards.
Schedule: 32 hours per week working 3:00 pm – 11:00 pm or 11:00 pm – 7:00 am (variable to these two shifts), minimum every 3rd weekend, call backup 10-15 times a year, up to 2 holidays per year
Key Job Duties:
1. Collaborate with primary nurse to help provide clinical and supporting case management based on patient’s diagnosis and end-of-life symptoms.
2. Complete initial, comprehensive, and routine assessments of patient and family needs.
3. Respond to urgent patient/family needs in collaboration with triage nursing staff
4. Initiate crisis care as needed
5. Develop, implement and review care plans for patient and family. Update care plan as the needs of the patient and family change
6. Report appropriate findings and significant changes to the primary team, attending physician and primary caregiver
7. Communicate with all interdisciplinary team members
8. Teach family members patient care skills and encourage their participation including, but not limited to, pain control, medication management, dressings, hygiene and nutrition
9. Provide education to patient/family members on disease progression, symptom management, medication and other associated patient care needs
10. Administer and titrate medication according to physician/nurse practitioner orders
11. Record all received orders to the Electronic Medical Record and initiate follow up as needed
12. Delegate nursing duties to LPN and supervise care provided by CNA
13. Prioritize daily work and make adjustments taking into account the needs of patients, Unity and other staff
14. Complete new patient admissions as needed

Unity’s Nurse Fellowship Residency Program provides all newly hired nurses, regardless of prior experience, with a thorough orientation that includes at least 10 weeks with a dedicated preceptor and a 9-month curriculum focused on building essential hospice skills and foundational knowledge.

Requirements:
Current unrestricted Registered Nurse licensure from the State of Wisconsin
Excellent critical thinking, interpersonal relationship building, and technical skills
Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage
