We are a rapidly expanding, physician-led primary care organization committed to transforming healthcare for seniors.
We seek innovative, ambitious, and compassionate Primary Care Physicians (PCPs) who are accountable for patient outcomes, providing comprehensive care for up to 450 patients.
Responsibilities include diagnosing, treating, managing acute and chronic conditions, leading care teams, and coordinating with specialists and hospital staff.
PCPs act as health coaches, simplify care plans, and promote behavior change, focusing on quality, patient experience, and cost-effective outcomes.
They must demonstrate leadership, evidence-based practice, cultural competence, and self-care.
Requirements include MD/DO with a valid license, board eligibility or certification, BLS certification, and ability to travel locally and nationally as needed.
