Job Description

We’re seeking a Primary Care Physician committed to transforming senior healthcare with an innovative, compassionate approach.

Responsibilities include providing comprehensive care for patients with acute and chronic conditions, managing a panel of up to 450 patients, and leading care teams to achieve quality outcomes.

Key duties involve assessment, diagnosis, treatment, health coaching, and care coordination, including engagement with hospital and transitional care teams.

The role emphasizes accountability, evidence-based medicine, leadership, and fostering a positive, patient-centered culture.

Requirements include MD or DO in Family Medicine, Internal Medicine, Geriatrics, or similar, active medical licensure, board eligibility or certification, BLS certification, and DEA registration.
