Job Description
The Pre-Transplant Coordinator is a registered nurse responsible for coordinating all aspects of transplant referral, evaluation, waiting, and peri-operative care.
Uses effective communication with the transplant team, patients, and providers to ensure comprehensive care and accurate updates.
Identifies and responds to clinical, psychosocial, and financial needs of patients during evaluation and waitlist phases.
Coordinates evaluations, provides education on transplant processes, and monitors patient status, ensuring compliance with regulations.
Documents and maintains data for regulatory agencies, manages insurance authorizations, and participates in quality improvement initiatives.
Requires a nursing degree, RN license, BLS certification, and relevant experience, with strong assessment, communication, and organizational skills.
The role involves on-call responsibilities and may include community engagement and continuous professional development.
