Job Description
The Post-Transplant Coordinator is a registered nurse responsible for coordinating all aspects of peri-operative and post-transplant care.
Communicates effectively with the transplant team, patients, and families, providing education and psychosocial support.
Follows patient-centered care standards, monitors patient progress, identifies complications, and collaborates on treatment plans.
Ensures compliance with regulatory standards, maintains accurate documentation, and manages data reporting.
Validates insurance, manages workflow efficiently, and contributes to quality improvement initiatives.
Participates in ongoing professional development, applies evidence-based practices, and demonstrates strong assessment and organizational skills.
Requirements include RN licensure, BLS certification, four years of nursing experience, and preferably transplant or ICU background.
