Job Title: Care Coordinator

Responsibilities:

• Provide care coordination for severely mentally ill adults and children with complex needs.
• Collaborate with an interdisciplinary team to develop and implement individualized care plans.
• Advocate for clients to access necessary medical, mental health, and substance abuse services.
• Engage clients and families in treatment, ensuring their voices are heard.
• Monitor client progress and adjust care plans as needed.

Qualifications:

• Bachelor’s degree in social work, psychology, or related field.
• Experience with mental health and substance abuse populations.
• Strong communication and advocacy skills.
• Ability to work collaboratively in a team environment.
