Job Title: Home Health Physical Therapist

Overview: Join our team in Downtown and North Jacksonville! This full-time position offers a competitive salary of $75,000-$85,000, paid on a per-visit basis.

Responsibilities:

• Conduct patient assessments and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility and function.
• Educate patients and families on home exercise programs and safety.
• Document patient progress and adjust treatment plans as needed.
• Collaborate with healthcare team members to ensure comprehensive care.

Qualifications:

• Licensed Physical Therapist in Florida.
• Experience in home health preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
