Job Description: Core Medical Group is seeking a dedicated Travel Home Health Physical Therapist for a 13-week assignment. The ideal candidate will provide high-quality therapy services to patients in their homes.

Responsibilities:

• Conduct patient assessments and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility and function.
• Educate patients and families on home exercise programs and safety.
• Document patient progress and adjust treatment plans as needed.

Qualifications:

• Valid state PT license.
• Experience in home health preferred.
• Strong communication and interpersonal skills.
• Ability to work independently.
