Job Description

We are hiring a Full-Time Physical Therapist for Home Health Visits! The Physical Therapist is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions.

Responsibilities:

• Conduct comprehensive patient assessments and evaluations.
• Develop and implement individualized treatment plans.
• Monitor patient progress and adjust treatments as necessary.
• Educate patients and families on rehabilitation techniques.
• Collaborate with healthcare team members to ensure optimal care.

Qualifications:

• Licensed Physical Therapist in the state.
• Experience in home health preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
