Job Title: Speech Therapist (PRN Home Health Visits)
Location: Jacksonville, Macclenny Area

Responsibilities:

• Conduct assessments and evaluations of patients with speech and language disorders.
• Develop and implement individualized treatment plans.
• Provide functional training in communication and swallowing techniques.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Document patient progress and adjust treatment plans as necessary.

Qualifications:

• Master’s degree in Speech-Language Pathology.
• Current state licensure and certification.
• Experience in home health preferred.
• Strong communication and interpersonal skills.
