Job Description

Job Title: COMPASS Care Navigator

Responsibilities:

• Manage a caseload of patients living with HIV enrolled in the Care Coordination Program.
• Collaborate with a diverse, multi-disciplinary team to provide comprehensive support.
• Assist patients in accessing medical care, social services, and community resources.
• Monitor patient progress and adjust care plans as needed.
• Educate patients about HIV management and treatment options.

Qualifications:

• Bachelor’s degree in social work, public health, or related field.
• Experience working with HIV populations preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team environment.
