Job Title: Master Social Worker – End Stage Renal Disease

Responsibilities:

• Assess patients’ psychosocial needs related to End Stage Renal Disease.
• Educate patients and families on rights, responsibilities, and care options.
• Collaborate with healthcare teams to develop comprehensive care plans.
• Provide emotional support and counseling to patients and families.
• Assist in navigating financial resources and community services.

Qualifications:

• Master’s degree in Social Work (MSW) required.
• Current state licensure as a social worker.
• Experience in healthcare or chronic illness management preferred.
• Strong communication and interpersonal skills.
