Job Title: RN Case Manager

Department: Case Management

Schedule: PRN

Location: Ascension St. Vincent’s Southside, Jacksonville, FL

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Coordinate with healthcare teams to ensure quality care delivery.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to patients and families.

Qualifications:

• Current RN license in Florida.
• Experience in case management preferred.
• Strong communication and organizational skills.
• Ability to work independently and as part of a team.

This is a PRN position and is not eligible for benefits.
