Seeking a Nurse Practitioner for PRN home risk assessments, with new grads welcome and a $1,500 sign-on bonus.

Conduct up to hour-long assessments reviewing health, medical history, medications, and social factors in patients’ homes or community settings.

Collaborate with physicians, case managers, and caregivers to provide comprehensive care and follow-up.

Perform assessments in various environments including homes and skilled facilities in Oberlin, Ohio.

Earn $1,900 to $9,500/month based on visit volume, with flexible days, evenings, nights, and weekends.

Qualifications include a Master’s degree or relevant experience, current RN/NP licensure, board certification, and ability to travel locally.

Benefits include mileage reimbursement and limited voluntary benefits; PRN providers do not receive benefits except 401(k).

Strong communication skills, computer proficiency, and flexibility are required. Travel and valid driver’s license necessary.
