Job Description

Seeking a Nurse Practitioner for PRN home risk assessments, with new grads encouraged to apply and a $1,500 sign-on bonus.

The role involves conducting comprehensive assessments in patients’ homes, skilled facilities, or community settings, reviewing health history, medication, social factors, and identifying care gaps.

Responsibilities include collaborating with healthcare teams, providing patient education, and documenting findings.

Compensation ranges from $1,900 to $9,500 per month based on assessments completed.

Requirements: Master’s or equivalent experience, current RN and NP licensure, board certification, CPR/BLS/ACLS, and travel ability.

Flexible hours, mileage reimbursement, and optional benefits are offered.

Location: Tiffin, Ohio, with travel required. New grads welcome; strong communication and computer skills needed.
