Job Description

Seeking a Nurse Practitioner for PRN home risk assessments, with new grads encouraged to apply and a $1,500 sign-on bonus.

Conduct comprehensive assessments in patients’ homes, clinics, or facilities, reviewing medical history, social factors, and health risks to improve care gaps.

Collaborate with physicians, social workers, and caregivers, providing follow-up and patient education.

Position offers flexible hours, days/evenings/nights, with potential earnings up to $9,500/month based on completed assessments.

Qualifications include a Master’s degree or relevant experience, current RN/NP licensure, board certification, CPR/BLS/ACLS, and valid driver’s license for travel.

Benefits include mileage reimbursement and 401(k); PRN providers are non-benefited.
